YMCA survey for families with school-aged children interested in a summer program

The YMCA is conducting a survey to see if there are families with school-aged children interested in a summer program.  This would be for both Ocean Shores and Pacific Beach areas.  There is a survey for both Pacific Beach and Ocean Shores, so please fill out the survey that corresponds with your location.
